I made a quickie form in Google Docs. This is a great tool for doing
assessments, surveys and short quizzes with students. I didn't spend a lot
of time on it, but I hope you can see its use. We will explore this in the
small groups next week.

A piggy-back on the form that Susan created:
Note the embed in gmail.
The form can be submitted directly from the message. (It may or may not in
other email apps). Very easy to use.
A form can also be setup so that "submitters" can see a synopsis of
responses (used more for staff functions than student).
Forms are based on spreadsheets that worksheets can added to that can be
manipulated to aggregate data. (grade an online test, for example).
The creator decides how much to share (which worksheets, etc.) for public or
collaborators.
Forms are a pretty powerful Google tool!

Here is a sample of the spreadsheet automatically created from the form I
sent out the other day.

I removed the names so as to not attribute the comments to anyone. This
was just a test soyou could see how the form feature worked from an
administration standpoint. I will delete the spreadsheet from my account
after this discussion session closes. Enjoy!
